Php Cant Insert Data Into Mysql by coliege sql

i tried your tutorial but i just cant insert data into mysql database.

the script i am using cannot enter the information in the form i dont know what i am doing wrong here

i followed each step accordingly to be able insert into the sql table using the username and password

can you help me find out why i am not able to insert the info into the data base.
